Taking Your Canadian Immigration Medical Exam in the US

Since August 21, 2025, Express Entry applicants and their family members must complete an immigration medical exam (IME) before submitting their PR application. Only an IRCC-designated panel physician can do it. There are panel physicians across the US, so you don’t need to travel to Canada. Book right after you receive your ITA. The clinic gives you an IMM 1017B Upfront Medical Report to upload with your application.

The medical is part of the 60-day post-ITA sprint, together with your FBI background check and proof of funds.

What changed in 2025

Express Entry applicants used to wait for IRCC to request a medical after they applied. From August 21, 2025, the exam became an upfront requirement. You complete it first, then upload proof with your application. If you’ve had an IRCC medical within the past five years, you may be able to give that IME number instead. IRCC will say whether you need a new one.

How to book in the US

  • Find a panel physician. Use IRCC’s “Find a panel physician” tool and select the United States. Only listed doctors are accepted; your own doctor can’t do the exam.
  •  Book for every family member. Your spouse and dependent children need exams too, even if they aren’t coming with you.
  •  Bring the right documents. Passport, glasses or contact lenses if worn, a list of current medications, and any relevant medical records.
  • Complete the exam. Physical exam, medical history, and age-dependent tests such as a chest X-ray, blood and urine tests.
  • Get the IMM 1017B. The clinic gives you the Upfront Medical Report. Upload it with your PR application.

Cost and timing

Panel physicians set their own fees, so costs differ by clinic and by which tests you need. Call several clinics for quotes. In the 60-day ITA window, book your exam in the first week. Results go electronically to IRCC, but follow-up tests (for example, after an abnormal X-ray) can add weeks.

Validity

Medical results are generally valid for 12 months. Your COPR is usually tied to that date, and you must land before the COPR expires. See landing as a PR from the US.

If something comes up

Most conditions don’t stop you immigrating. IRCC assesses public health, public safety and excessive demand on health services. If a condition is flagged, you may receive a procedural fairness letter. Respond carefully. Read our guide to medical inadmissibility.
